Step 1 [Installation Steps]
It's recommended that you create a new AWeber Campaign for each quiz landing page you install. Below is the screen shot for adding a new list.

Set your list's "Reply Address". It's highly recommended that you use an email address from the same domain that your quiz is hosted on.

From the "List Settings" tab, select "Web Form" to generate your AR Form Code.

Click the green "Create Web Form" button.

In "Form Details", make sure that "Type" is set to "In-Line".
The "Already Subscribed Page" prevents AWeber from simply displaying an error if the quiz-taker is already on your quiz list. In that case, the script treats the request as "confirmed" and "onlist".
Click the green "Next" button when complete.
The new version of aweber changed the interface, so you will see something like here for the thank you page and for the on list page:
For the thank you page, chose the custom page:
Enter the URL given for Thank You page, from the Optin Quiz Generator, Section 3.1 and copy it where below it says: Enter Your URL
For the on list (or now called already subscribed page), chose the Custom Page:

Step 6 [Installation Steps]
Drag the "Name" field over to the Preview window and place it above the "Email" field. Click on the "Submit" button and change the text to "Send my analysis".

Click the green "Save" button when complete. Click the "Get HTML" hyperlink to view the AR form code.
Copy and paste this AR form code into a text file. We'll use it in a few moments.

Under the "List Settings" tab, click on "Confirmed Opt-in". You can customize your confirmation email text on this page, but the setting we're most interested in is below.

When using double opt-in (and you should be), the "Confirmation Success Page" is necessary to set. This is the page that AWeber will redirect your quiz-taker to AFTER they have confirmed their opt-in.
This page is given by you from the dashboard, just copy and paste. It is the page in the OQG script that triggers the analysis email to be sent or displayed in the web browser.
Step 10 [Installation Steps]
We now want to take the AR form code we saved in our text file, and copy the form code into the Opt-in Quiz Generator script, Step 3.1 of the dashboard as shown below.
You can choose between different types of optin solutions. In the case of No Optin, the quiz taker will not need to insert the name/email to get the analysis but will be automatically displayed on the next page.
If you chose No Optin a new window below allows you to place an email address of yours that will receive a copy of the analysis, you chose so.
The single optin and/o double optin choice is self explanatory.
Ensure you select the "Double Optin" radio button, "AWeber" from the Auto Responder drop down list, and then simply paste the code into the Auto Responder Code box.
